Creating simple request types with a radio button approach

We have some simple request types that just require the user to submit a simple request. For instance HVAC, would be good area to have a request type where the user could select between too hot or too cold. In settings > request types > manage request categories as we're configuring categories in the description area tiles or radio button they could select for their issue.
